I declare that the information provided in this Security-Related Pre-Arrival Information (SR-PAI) form is true, complete, and accurate to the best of my knowledge and belief.

I understand that: (i) false or misleading declarations constitute an offence under the SPOMO Act 2019 and applicable maritime security laws, and may result in administrative sanctions, denial of port services, and/or criminal prosecution; (ii) this information will be processed by the NIMASA ISPS Ship Unit and shared with NPA, MSSSD, and MEM for vessel clearance purposes; (iii) this submission does not substitute for the Declaration of Security (DoS), which is a separate instrument executed at the port facility.
